R. Yaakov ben Moshe Levi Moelin (1365-1427) is best known for his codification of the Minhagim Ashkenazic Jewry which served as one of the primary sources for the Rema in his glosses on the Shulchon Oruch.
Printed in Slopkowitz, where only 5 seforim were published and where many students of the Ba’al Shem Tov resided.
